West of England Polypharmacy share and learn webinar: managing diabetes

Our upcoming eigth Polypharmacy Community of Practice is an opportunity to share learning with peers related to pharmacological and dietary management of diabetes.

We have Senior Pharmacist Pam Bhambra, to present her project working with frail elderly diabetic people in care home settings and Senior Medicines Optimisation Pharmacist for Diabetes, Louise Ellis. Louise will support further discussion with her presentation on approaches for managing diabetes and polypharmacy.

Topics covered will include:

• Working with Care Homes to support diabetic residents
• Effective insulin management for older / frail people
• Challenges of changing / improving dietary intake

Please come along to learn from peers and share your experience working with people with diabetes.

The session offers an opportunity for all clinicians interested in the health and wellbeing of frail elderly people that may have an interest in prescribing for diabetes, whilst reducing polypharmacy and taking a holistic approach to person-centred care.
