Our collaborative is comprised of the Health Innovation Networks for the South West, West of England and Wessex and our approach is supporting colleagues in Primary Care, within communities across the region to improve early detection and subsequent treatment optimisation of patients with CVD and Heart Failure, in order to:

  • Improve population health, particularly in underserved communities
  • Enhance the promotion of healthier lifestyle choices to help people live longer and healthier lives, positively contributing to the region’s economic growth
  • Reduce emergency department attendances and acute admissions in all hospitals in the region, to help with ongoing capacity challenges
  • Identify opportunities to introduce new innovations that support earlier diagnosis, timely initiation of effective treatment and patient self-management.

Through working collaboratively, the South West CVD Collaborative is maximising impact for the region through sharing approaches and resources, exchanging knowledge & learning, aligning strategies with clinical networks, and avoiding duplication of efforts.

CVD Connect Newsletter
