The West of England AHSN has been announced as one of the AF Association “Healthcare Pioneers 2018 – Showcasing Best Practice in AF” winners at the Arrhythmia Alliance Awards Ceremony held at the International Heart Rhythm Congress for Don’t Wait to Anticoagulate – our quality improvement approach to help prevent atrial fibrillation (AF)-related strokes. We have been working in collaboration with Gloucestershire Clinical Commissioning Group (CCG) and Bayer Healthcare to prevent strokes amongst patients with atrial fibrillation by improving medicines management in primary care.

The video below gives an outline of the project:

Trudie Lobban MBE, Founder and CEO, AF Association on congratulating the 2018 Award winners stated, “The AF Association Healthcare Pioneers Award, is presented to examples of truly innovative best practice covering identification, diagnosis, management, treatment and care of patients with atrial fibrillation (AF), which we summarise as detect, protect, correct and perfect.  Each year we share the published Healthcare Pioneers report with commissioners across the NHS urging them to use these case studies as a benchmark to drive improvement in processes and patient pathways for the diagnosis, treatment, and care of AF patients.”

“This year we had an exceptional number of entries, the highest number we have ever received and all were of an excellent standard, demonstrating tremendous innovation to improve the lives of AF patients. We thank everyone who entered, and especially commend those chosen as this year’s Healthcare Pioneers Award winners,” she added.

Don’t Wait to Anticoagulate includes toolkits for clinicians, pharmacists and patients to help prevent atrial fibrillation (AF)-related strokes.

These toolkits are available via the Don’t Wait to Anticoagulate website, funded through a joint working project between Bayer HealthCare and the West of England AHSN. The resources were co-designed with a wide range of stakeholders, including NICE, patient representatives and clinical partners.

The project also won an award at The Anticoagulation Achievement Awards in October 2017.
