Medicines Breakthrough Collaborative 1

This will be a fantastic opportunity to learn about Breakthrough Series Collaborative Models of Working and get involved in the medicines optimisation programme which aims to deliver improvements in insulin safety and deliver improvements in medicines at the transfer of care.

Our aim is that by working together (with patients and each other) we can deliver safer and better outcomes from medicines at transfer of care in the domains of patient safety, patient outcomes and patient experience for people in target population.

Please state when registering which group you would like to be part of: transfers of care or insulin safety.

This is a collaborative programme working across all sectors in the West of England AHSN region, and includes patient involvement and industry partners.

This event would be of particular interest to diabetes specialists and staff involved in medicines optimisation and reconciliation processes when patients transfer between care settings, as well as patient representatives.
