This Local Polypharmacy ALS leverages an evidence-based learning model, developed by Wessex AHSN in partnership with Health Education England. Its core objective is to equip GPs, pharmacists, and other healthcare professionals engaged in prescribing, medication reviews, and de-prescribing with the knowledge to navigate the complexities of safely discontinuing unsuitable medications. Additionally, this ALS supports Primary Care Networks in implementing the Medicines Optimisation facets of the primary care contracts, such as DES and QOF, aligning with recommendation 18 of the National Overprescribing Review report.

The programme unfolds across three 90-minute online interactive sessions over two months, utilising Teams:

Session 1: 19 September 12- 1:30pm Polypharmacy – Unpacking the magnitude, implications, and obstacles of safely reducing and stopping medications. We’ll clarify what constitutes inappropriate medicines and delve into the patient perspective.

Session 2:  3 October 12- 1:30pm Available Tools – Investigating shared decision-making, alongside the evidence and resources essential for conducting thorough medication reviews.

Session 3: 17 October 12- 1:30pm Workshop and Facilitated Discussions – Engaging with a consultant geriatrician to exchange experiences on medication reviews and consolidate learning.

Please note:

  •  We review registrants to ensure you meet our delegate criteria prior to final confirmation of your place.
  •  All delegates are required to attend all 3 sessions and participate in an evaluation to obtain the certificate.
