Event summary
Health Innovation West of England welcomes Senior Pharmacist Pam Bhambra, who will present the outcome data for her project working with frail elderly diabetic people in care home settings, alongside Senior Medicines Optimisation Pharmacist for Diabetes, Louise Ellis. Louise will present on Bristol, North Somerset and South Gloucestershire (BNSSG) Diabetes Frailty guidelines to inform and support managing diabetes and polypharmacy for frail patients.
Topics covered will include:
- Working with care homes to support diabetic residents
- Effective insulin management for older / frail people
- Challenges of changing / improving dietary intake
